Eligibility Requirements

Patient has to be a current University of Waterloo undergraduate or graduate student, or family member (dependents – spouse/children; their parents and visiting family and support people)

Health card, third party insurance, this is discussed based on individual needs of client.

Traditional, straightforward, intake process. Patient will be asked to sign a consent form, will be asked why they are there, meet with physician that best suits students needs.

If you need a female or male doctor, what you’re looking for in regards to your care.

Things To Note

Shared electronic health record keeping to promote collaboration. Clients can consent to that. Promote client directed care.
